- Physical exam from a health care provider
- A Goodwin University Physical Form will be provided to applicants who pass the TEAs exam. Only this form will be accepted.

- Influenza vaccination: All students in the program must receive a flu vaccine annually No exemptions are permitted.
- COVID-19 and booster: Submit documentation of your COVID-19 vaccination(s) (1st and 2nd). If your series is in process, submit your first vaccine and a new alert will be created for you to provide your second vaccination, if applicable. Documentation must include the vaccine manufacturer and Lot#. No exemptions are permitted.
- Cardiopulmonary Resuscitation (CPR) certification: The only 2 courses accepted are Healthcare Provider BLS (American Heart) and Professional Rescuer BLS (American Red Cross).
- Background check/drug screen: All students accepted to the LPN program must submit to a background check and drug screening. Goodwin University covers the cost through the Navigators Scholarship for LPN certificate. However, if the drug screen result is “negative dilute,” the student must assume the cost of a second drug screen. A positive drug screen, not substantiated by a healthcare provider, will prevent the student from entering the program. Students will be notified if the background check reveals any criminal record that might interfere with future licensure. If a clinical facility prohibits the student from participating in a clinical experience based on this result, the department will make two additional attempts to place the student in an alternative facility. If neither of the two further attempts yields a placement for the student, (s)he will not be permitted to progress with the program.
Any questions regarding documentation should be referred to the clinical coordinator Sue Jahn at .
