Personal Leave of Absence
In the event that you wish to take a leave of absence and your circumstance does not qualify for coverage under any of the previously-mentioned leaves of absence, you may request permission, in writing, from your management team. The letter must be addressed to your manager, specify the reason for the leave, and indicate the start and end dates of the requested leave. Your manager will review the request with the VP, EVP/Provost and Human Resources to determine the decision. Please note that approval of such requests is extremely rare, may affect your eligibility for benefits, and will not guarantee reinstatement of your position upon return.
